How to Create and Publish an Enquiry Form in Bookiphy?

Collecting new leads and enquiries from your website or social media has never been easier. With Bookiphy, you can create, customise, and publish a fully branded Enquiry Form in just a few clicks — and automatically track all submissions in one place.


Step 1: Create Your Enquiry Form

  1. Go to your Bookiphy Admin Dashboard.

  2. Navigate to Forms → Create Form.

  3. Choose Enquiry Form from the available templates.

  4. Add your Club or Academy name, upload your logo, and include a short welcome message to describe your services.

    Tip: Keep the description short and informative — this is what parents will see at the top of your form.
  5. Enter your contact details, including:

    • Phone Number

    • Email Address

    • Location

    • Website (optional)


These details will appear at the top of your form to make it easy for parents to contact you.


Step 2: Add and Customise Form Fields


Your enquiry form automatically includes all the key fields needed to capture leads efficiently.


Contact Details

  • Contact’s Full Name

  • Email Address

  • Contact Number

  • Contact Relationship (Parent / Carer / Guardian / Relative / Other)


Student Details

  • Student’s Full Name

  • Date of Birth or Due Date

  • Desired Start Date

  • Desired Enrolment Year

  • What are you looking for? (a short text box where parents can describe their request)

You can also edit field labels or hide optional sections under Form Settings.


Step 3: Personalise Your Form


Once your form is created, you’ll find several options in the right-hand panel:

  • Change Description – Update your intro text or service details.

  • Edit Contact Details – Modify your phone, email, or location anytime.

  • Change Button Name – Customise the submit button (e.g. “Submit Enquiry”, “Send Request”, etc.).

  • Thank You Message – Add a personalised confirmation message for parents after they submit the form.

  • Update Form Settings – Control field layout, design, and visibility.

  • Share Form – Copy your form’s link or get the embed code for your website.


Step 4: Publish Your Form

When you’re happy with your design:

  1. Click Publish Form at the top of the setup wizard.

  2. Once published, you’ll see the status as ? Published.

  3. You can now share or embed your form anywhere.

Example:
https://bookiphy.com/funclub/enquiry
(Your unique link will be auto-generated based on your club name.)


Step 5: Share Your Enquiry Form

From the Share Form option:

  • Copy and share the form link on your website, WhatsApp, or social media.

  • Or use the embed code to display the form directly on your site — so visitors can submit enquiries without leaving your page.


Step 6: View and Manage Enquiries

Once your form is live, every submission will appear instantly in the Enquiries section of your dashboard.


Manage Enquiries

You can view all incoming enquiries in a simple table format with:

  • Student Name

  • Date of Birth

  • Contact Name

  • Email & Contact Number

  • Comments or Questions

  • Tentative Start Date & Enrolment Year

  • Status (Open / Closed)


You can also:

  • Use the search bar to quickly find specific entries.

  • Click the three-dot menu (…) under Action to edit, update, or mark an enquiry as closed.

  • Use the Add Enquiry button to manually record offline or phone-based enquiries.


All enquiry records are stored securely in your Bookiphy dashboard — giving you a central place to track leads, follow up, and convert them to bookings.
